2019-20 Kindergarten Registration
WHO: Rising kindergartners - children who will be five years old on or before September 1, 2019 - and their parents or guardians.
WHY: The first day of school is Monday, August 5, 2019. (Look online for other important calendar dates for the 2019-20 school year.
HOW: See below
Step 1: Complete Kindergarten Online Registration-
Click here to complete online registration. Registration opens April 1, 2019.
Step 2: Gather necessary documents:

Proof of Date of Birth. Examples include an original birth certificate or birth registration, an official state or federal form with date of birth, or passport. Parents can order a birth certificate for a child born in Georgia by visiting the Georgia ROVER website at https://services.georgia.gov/gta/rover/

TWO different proofs of residency in attendance zone.

1- A current utility bill (excluding telephone bills) AND one of the following documents:

a non-contingent sales contract

current lease/rental agreement

most recent income tax return
-
current paycheck stub
-
current residential property tax statement or bill

current warranty or quitclaim deed

current home purchase agreement
-
current homeowner’s insurance policy

Documents presented for residency verification must include the same address as the accompanying utility bill. An exception: A deed without an address is acceptable if accompanied by two utility bills (excluding telephone bills) with same address in the attendance zone. A contingency contract is not acceptable. If you live with a friend or family member and need information about verifying your residency, call the Registrar at Dacula ES.

Georgia certificate of immunization (Form #3231) or a notarized Affidavit of Religious Exemption (Department of Public Health Form #2208). Form 3231 must be marked with either "Date of Expiration" or "Complete for School Attendance".

Vision, Hearing, Dental, and Nutrition Screening (Form #3300) dated within the last 12 months.

Your photo ID- Examples: a driver’s license, a state identification card, a passport, or other official photo identification, such as an ID card obtained through an official government agency or consulate.

The child’s social security number.
OPTIONAL- Step 3: If you would like to participate in our FAST PASS screening, please stop by the school with your child's paperwork any day before May 2nd, between 9:00 am-2:00 pm. Once all your paperwork has been turned in, you will be able to select a time to bring your child in to complete their portion of registration.
Step 4: Bring your child and all paperwork to DES on May 2nd between 12:00-6:00.

Alternate Transportation Form
Any student going to a daycare or other address (not their home) every day, will need a completed Transportation Parent Authorization (Alternate Bus Form). Click Here for the form.
